2025 Form 1095-C Available

Form 1095-C is a tax document provided by employers with 50 or more full-time employees to report information about  health insurance coverage offered, as required by the Affordable Care Act (ACA). If you would like to receive a copy of your 1095-C, please contact us at hr@willamette.edu.
